Derek F. Clark New Castle Scholarship

Requirements: 3.0+ GPA, Extra-curricular activities

Description:

This scholarship was established in the fall of 1989 in memory of Derek F. Clark, who died at age 14 following a nine-year battle with leukemia, and was an inspiration to the entire community. Throughout his life he continued to excel—as a student, as an athlete, as a student leader, and as an active church member. The winner of this scholarship must exemplify those four areas: scholastic achievement, athletic achievement, community involvement, and extra-curricular activities in high school.

CRITERIA:
graduating senior(s) from New Castle High School
Accumulative GPA of 3.0
Enrolled in an accredited college or university on a four-year program.
Participation in Athletic events for all four years of High School
Community involvement
Extra-curricular activities in High School

REQUIRED ATTACHMENTS IN THIS ORDER (no staples):
1. General Application
2. Personal Data
3. Lists of:
  a. Community activities and involvement
  b. Athletic activities including years of involvement
  c. Extra-curricular activities in High School
4. Transcript
5. College Acceptance Letter
6. Two Recommendation Forms (from High School Principal, High School Guidance Counselor, Teacher, Minister, Employer, etc.)

SCHOLARSHIP APPLICATIONS WILL NOT BE CONSIDERED IF:
1. Application is incomplete (missing any of the above attachments)
2. Application is not arranged in the above stated order
3. Application is stapled
4. Application is not received at the Foundation office by the due date: March 11, 2013 at 5pm

OTHER INFORMATION:

Financial need is not a priority.

Scholarship Award Range- $$

If student does NOT attend a university or college, all money awarded shall be reimbursed to the scholarship fund.

Payments will be paid in two installments, the first in the fall semester, the second in the spring semester after the student has turned in grades to the Foundation Office.

IMPORTANT:  The Henry County Community Foundation will not send checks to the college choice until a thank you has been received at the foundation, addressed to the individual provided on the award letter.  The foundation will mail the thank you note/card to the appropriate individual.
